Here is a patient of mine who had multiple old hair transplants harvested via FUT from another surgeon. He has a severe form of hairloss (norwood 6) and the surgeon gave him a very aggressive hairline (which is why I always do a more conservative hairline in younger patients). His results ended up being very unnatural with massive hairloss happening behind the hairline. The gap between the hair on the bottom sides and the bald areas on top (the alleys as I call them), is a give away for a hair transplant. Also because he had extensive scarring from his previous procedures he had very little donor hair left in the back. I harvested 1613 grafts via FUE primarily from the sides which still had decent density and transplanted extensively into the alleys and the mid scalp and crown. The results are impressive and much more natural than before. Also we are going to do another procedure in the future using neck beard hair, which will also increase the density even more. The patient is thrilled.

Here is another one of our FUE hair transplant patients showing his result approximately 1 year after his procedure. As you can see the results are very natural. Something that I try to convey to prospective patients is the fact that not all hair transplants are not created equal. This is living art, and as such every artist will create a different hairline, with different densities of hair, different angles and orientations of the transplanted hair, etc. There are so many variables that make the difference between an excellent result, versus a decent result, versus a bad result. There are also many variables in the donor harvesting that can diminish or ruin the donor zone for the future, versus preserving as much donor hair as possible for future procedures and taking out only the grafts necessary for the current situation. Many clinics don’t expect the ever see the patient again and take most or all of their donor hair in a single procedure. This is extremely shortsighted as hair loss is a continuing process and its very important to evaluate any prospective hair transplant patient not just what they look like today, but to predict their endpoint of hairloss to the best of our ability and then determine how many grafts will be required over time to address current loss, and also keep up with continuing loss. This is why I perform every hair transplant consultation personally and don’t have “consultants” performing these on my behalf. I want every patient to be evaluated by myself  and to hear realistic expectations for the  short term and long term from me personally. I also want the opportunity to examine the patient and get their hair loss history so that I can have a good idea about future loss as well. This is also why I personally extract every hair follicle from the donor zone as well, while many other doctors delegate this to technicians. This is a crucial step both in getting intact follicular units for a successful hair transplant, while also preserving as much of the donor zone as possible for future procedures. Before choosing your hair transplant surgeon/ artist, make sure you have seen many hundreds of before and after photos by that surgeon that look aesthetically pleasing to you. Also make sure your surgeon is seeing you in the consultation and performing the harvesting of the hair personally. Finally, make sure your surgeon has a singular focus in hair restoration and doesn’t have a menu with many different cosmetic procedures and hair is just one item on the menu. There is a reason why specialists do nothing else but hair restoration, and to be great at something is to do that same thing many times over.

Here is a patient of mine who had a Mohs surgery to excise a basal cell lesion in the scalp. It unfortunately resulted in a large scar in the frontal scalp and when the operating surgeon attempted to minimize the scar it actually got significantly worse.

The patient presented to me extremely depressed over the large scalp defect just behind her hairline. I transplanted approximately 1000 FUE grafts in two procedures spaced 8 months apart.

The patient is thrilled with her results and this has been a life changer for her. She can now style her hair normally and go out without fear of a heavy wind exposing a large bald patch in the front.

I am often asked if hair can be transplanted into scars and the answer is yes. There is a lower growth rate through scars and we are not able to transplant as dense in a single procedure due to the compromised blood supply, but when performed correctly these cases can be very successful.

FUE hair transplant into scalp scar

FUE has become more popular than ever given the fact that in the hands of a skilled surgeon the results can be as good as FUT (The strip harvest method) or even in some cases better (due to the fact that with FUE we can choose which follicles to harvest and therefore choose the follicles with the most amount of hairs per follicle). Up to now one of the downsides of FUE was that it required the entire back and sides (harvest zone) to be shaved in order to harvest the follicles. Now we have a new way to perform FUE cases without shaving the entire donor zone. I call it “No Shave FUE”. What we do is go through the donor zone and cut short individual follicles that we plan to harvest and leave the surrounding hairs at their normal length. This makes the donor zone appear untouched to the naked eye, and is much less visually impactful than a head shave, and gives us the same results as a traditional FUE shaved case. The no shave technique is more time consuming and thus more expensive, but for those people (like myself) who refuse to shave the back and sides of their heads but want the FUE procedure, this is a great option. Below you can see an unshaven FUE patient one day after just over 2000 grafts were extracted via FUE.

Typically FUE has been performed by shaving the entire back and sides of the scalp as the hairs harvested via FUE need to be very short. Recently I have been performing no shave FUE or long hair FUE. This technique involves cutting short the hairs to be harvested from the donor region individually while leaving the hair around it long. This allows us to perform FUE without shaving the entire back and sides of the scalp and makes the entire process much less visually invasive. This process requires very specific expertise but for many people that want FUE but don’t want to shave their heads this is a great option. I recently posted a video of the process on my youtube channel which shows the entire process.

In the past we needed to shave the back and sides of the scalp to perform FUE because the follicles can only be extracted as short hairs.

Noe we have a new technique called long hair FUE. With this technique we go through the donor region and individually cut short the follicles we will extract while leaving the hair around it ling. This allows the patient to keep their hair long for the procedure and creates no significant change to the patients visual appearance.

This technique is a but more expensive than short hair FUE but for those that don’t want to shave their head it can be the difference between doing the procedure and not doing it.

For a long time the only way to perform FUE was to shave the entire back and sides of the scalp as the hair needs to be a few millimeters long in order to extract. That has now changed.

Now we can perform long hair FUE which allows the patient to keep their hair long and completely conceal any evidence that FUE was performed in the donor region.

The patient leaves their hair at least 1.5 inches long and we go through the donor hair individually and randomly cut short the follicles we plan to extract. This allows us to completely conceal the donor region. It also allows up to choose the follicles with the most hairs per follicle which gives the patient the best ultimate result.

Since I have had multiple strip harvest hair transplant procedures in the past, I wanted to have this technique performed on myself to better understand what it felt like, which would in turn give me greater insight into the process. I can wholeheartedly state that this process was so much easier than my previous strip harvest procedures. I had virtually no pain the next day and was back to work a day later.

I believe that this process will revolutionize FUE and open it up to so many more people that were hesitant, like myself, because they did not want to shave their head for the procedure.

This is a patient who previously underwent an FUE procedure performed by another physician with unnatural results secondary to the placement of large grafts in the hairline. I created more density and placed single hair grafts into the hairline to create a more natural look. The grafts were harvested via FUE using a 0.8mm motorized punch tip. Approximately 400 single hair grafts were placed into the hairline to create a much more natural look.

Unfortunately there are many physicians offering hair transplants that are not properly trained. It is imperative that you do your research before deciding on your surgeon and always chose someone who specializes exclusively in hair restoration.
