Here is a patient of mine who received a hair transplant from another physician over 10 years ago. The grafts then were placed primarily in the hairline. The patient lost more native hair and presented to me with a thinned out hairline and very little hair in the mid scalp region. I transplanted just over 2000 grafts via FUT to increase the density in the hairline and the mid scalp.

The “after” photos are shown approximately 1 year after the patient’s procedure.

The patient is thrilled with his results.

Here is a patient of mine who just returned for his 9 month follow up after receiving 1372 grafts to the hairline via FUT. The objective was to fill in and define the existing thinning hairline.

The patient is already thrilled with his results and he should see some additional cosmetic density over the next 3-6 months as the transplanted hairs thicken in diameter.

Here is a patient of mine who wanted empty patches in his beard and mustache filled in. I used a 0.8mm FUE punch to extract hair follicles from the neck and transplanted these follicles into the beard and mustache. The results are shown after 6 months and the patient is thrilled.

Here is a patient who recently presented to me with these strip scars from another physician. The patient had multiple strip hair transplant procedures and on the last procedure these very wide scars formed. I do not know exactly what the physician did, or how the patient presented to the physician prior to the last procedure, but this should serve as a warning to both physicians and patients that this can happen if you are not careful. Each patient must be throughly evaluated to determine elasticity, especially after multiple procedures. Each time a strip is harvested the donor elasticity decreases, thus increasing the possibility of scar widening, or the inability to close the would. Tension on the would closure also increases the chances of shock loss of hair around the donor region. Many times the hair lost from shock loss comes back over time, but sometimes it does not.

Sometimes you don’t know you’ve gone too far, until you’ve gone too far. Patients will push the physician in some cases to go for more grafts. Physicians  will want to make the patients happy and feel confident in their abilities. No matter how good you are, you must always be cautious and prepared for the unknown. It’s always obviously better to stop before you get to a problem.

With my success performing FUE I have been encouraging my patients who have had multiple strip procedures to consider an FUE procedure which will not affect a previous strip scar. If I feel it is medically permissible to harvest another strip, I always try to educate the patient that there is always the possibility of scar widening or shock loss around the donor region.

In this case, the treating physician tried to resect the scar with no success. I opted to treat the patient by extracting FUE grafts from the beard and transplanting them into the scars. I hope to post a follow up photo in 6 months showing the results of my procedure.

I am often asked what you look like 7 days after an FUE hair transplant so here are the images.

This patient received just over 1500 grafts to the frontal and mid scalp via FUE using a 0.85mm extraction tip.

As you can see with this small extraction tip the donor zone heals very well and there is basically no sign of any harvest after 7 days.

Here is a patient of mine who received old hair transplants with larger grafts from another physician many years ago. He came to me requesting greater density and more irregularity in the hairline to create more naturalness. He was not a candidate for another strip procedure and he had very limited donor reserves on his scalp.

I extracted approximately 600 grafts from the sides of his scalp via FUE and just over 600 grafts from his neck via FUE as well. I used a 0.8mm punch tip for my extractions so that the extraction sites would not be visible. The neck grafts were placed in the frontal and mid scalp for greater density and the scalp grafts were placed in the hairline to create a more natural look overall.

The results are shown after 12 months.

Beard hair FUE is an excellent option for patients who do not have any scalp hair available and this case demonstrates that very well.

I recently saw a patient in consultation who had received 3 previous hair transplants from another physician and wanted to be evaluated for an additional procedure from me.

On examination of his donor area I was surprised to  find 3 separate strip scars. One for each procedure, stacked on top of each other. The effect was significant scarring in the donor area and an inability to harvest another strip scar secondary to the extensive scarring from the previous procedures.

For reference I have also attached a photo of one of my patient’s who had 2 strip procedures, but only one scar. This is because whenever I go back for additional procedures on a patient, I always attempt to harvest the new strip to include the old strip scar. In essence instead of creating train track multiple stacked scars, I keep the scar as a singular scar, thus minimizing the overall scarring to the donor region. In addition, I also performed a trichophytic closure in my patient, which allows the hair to grow through the scar, thus further concealing the scar.

It is imperative to treat the donor region with as much care as the recipient region. Donor hair is finite, and proper care and attention must be given to ensure that we can minimize donor scarring and maximize donor harvesting over the patient’s lifetime.

Today I want to talk about something that everyone experiences to some extent. Pre-hair transplant procedure anxiety. I have undergone 3 procedures myself, and know exactly what to expect, and yet I still experienced some degree of anxiety prior to each procedure. Everyone is different and comes into this procedure with a different perspective. Some are more excited and less nervous and some are the other way around. I am very sensitive to this in my interactions with my patients. I will always do everything in my power to re-assure the patient and let them know that they are in great hands. I will also give my patients as much information as possible regarding what to expect throughout the procedure and post procedure time period. In addition, unlike many other clinics that perform hair restoration procedures, we are not trying to “sell” our services. That means starting with myself, who performs each and every consultation without the assistance of a “patient advisor”, “counselor” or “sales person”, my job is to educate the prospective patient, answer their questions, and give them realistic expectations as to what they can look forward to if they decide to undergo the procedure.  In no way and at no time will I or anyone in my practice pressure the prospective patient into moving forward with the procedure if they are not comfortable with it. I would prefer that a patient not move forward if they are not completely comfortable with all the aspects that it entails. Again, I can educate, help to alleviate irrational fears, and provide moral support throughout the process. I can also attest to the fact that almost every single patient of mine tells me a year after their procedure that their only regret is not having the procedure performed sooner and enjoying the results for longer. But at the end of the day, the prospective patient needs to be psychologically ready to move forward.
