Today I want to discuss a very new exciting development. For the past year I have been prescribing a compounded topical prescription formula that contains Dutasteride, Finasteride, Minoxidil, Retin A, and Triamcinolone. Dutasteride and Finasteride (propecia) are potent DHT blockers. Minoxidil is a vasodilator which has been proven to help genetic hair loss. Retin A helps with absorption and triamcinolone helps with scalp irritation. I call the formula “hair defense” as it’s defending the native hair. It does not grow back dead follicles that have been lost. It does however prevent susceptible follicles from falling out, and can turn miniaturized follicles (follicles that are on their way out and have become cosmetically insignificant) back into healthy terminal follicles, which can then give the appearance of greater density.

Below, you can see examples of patients of mine who haven been on the topical at least 6 months and all the photos shown below are just with the topical only as treatment.

This is not a substitute for a hair transplant procedure. Rather it is a an adjunct meant to preserve the native hair so that the results of the hair transplant procedure look better for longer. In some cases it can be used as an intermediate treatment meant to slow or stop the continuing loss before the transplant is indicated.

Finally, one of the best things about the topical is that the incidence of sexual side effects (though low in frequency can still occur in 1-2% of patients taking oral finasteride) are even more uncommon with this topical as the systemic absorption of the medication is less than the oral medication.

I have seen a profound impact of this topical medication on my own scalp and hundreds of my patients.

For a small number of patients that may get irritation from any of the components, the formula is customizable and components can be pulled out in any amount.

Here is a patient of mine who had micro blading and tattoos done to her eyebrows because she lost most of her natural eyebrow hair due to over plucking and genetic factors. She was unhappy with the results of the eyebrow tattoos and decide to undergo an eyebrow transplant procedure. The results shown here are after a single FUE eyebrow  transplant and approximately 250-300 hairs were placed per eyebrow. The patient, who happens to be a registered nurse herself,  is thrilled with her results and shares her experience  on this video.

Here is a patient who went to a hair transplant surgeon in Florida and had an ARTAS Robotic FUE procedure. The photo below shows how his donor looked after only about 1300 grafts were extracted by the robot. The donor area was very scarred as you can see from the photos. I harvested just over 1300 grafts with a handheld FUE tool in the areas that were left untouched from the previous procedure and the results are shown here.

Bottom line is choose your physician wisely and choose a physician who personally extracts the follicles by hand and focuses exclusively on hair transplants in order maximize chances for a successful outcome and natural result.

Here is a patient of mine who had numerous FUT strip harvest procedures by other physicians that resulted in severe scarring in the donor area, plug grafts throughout the scalp, and an overall very unnatural appearance. The result was so unnatural that the patient would not go outside without a hat.

I extracted approximately 1500 grafts from the neck via FUE in multiple sessions and placed them throughout the scalp, especially in the “alleys” on the sides to connect the transplanted hair to the native hair on the sides, and the crown in the back.

The patient also had some SMP performed on his scalp and will have more in the future.

The result is already so much improved from prior and the patient still has some more beard donor hair and will have more SMP to improve the result even more. I will also be “de-bulking” some of the plug grafts to make them more natural  looking.

Neck and beard hair can be a very valuable source of donor hair for those that are out of scalp donor hair. Though the character of the hair is a little different, it works very well as filler and can make a nice impact in improving visual cosmetic density. Also, the neck heals so well from the small punch sites of the FUE removal that the tiny dots are barely visible on close inspection.

Choose your surgeon wisely so that you end up with an aesthetically natural result both in the short term and long term.

Here is a result of a male patient of mine who received approximately 350 grafts to each eyebrow harvested via FUE. The results are shown approximately 1 year after the procedure.

Eyebrow transplant procedures are becoming more popular amongst male and female patients with weak eyebrows secondary to hereditary issues, scarring, or over plucking. The results can be very natural looking when

Here is a patient of mine who received an FUE hair transplant to the frontal scalp. This patient is a professional athlete who also wears his hair very short on the back and sides so an FUE procedure was required to fulfill his needs.

African American hair is typically much more difficult than other types of hair to extract with FUE due to the extreme nature of the natural curl. However with my technique and tools I have had great results performing FUE hair transplants on African American patients.

Here is a male patient of mine who has a case of LPP which is a form of scarring alopecia. He has lost small patches of scalp hair in no specific distribution secondary to the disease process and he lost all his eyebrow hair as well. He previously had an eyebrow tattoo and was disappointed with the look and presented to me for eyebrow transplants.

I explained to the patient that scarring alopecia is a very unpredictable process and that no guarantees could be made as to the success of the eyebrow transplant. Due to the fact that he had scattered patched affected in the scalp I felt our greatest chance of success was to perform the procedure with an FUE harvest and to find unaffected areas of the scalp to harvest the donor hair from.

I personally harvested every follicle using a 0.8mm motorized punch. We only placed single hair grafts into the eyebrows. We placed approximately 270 hairs per eyebrow.

The results here are shown after only 6 months. The patient should see some increased growth over the next 2-3 months.

The patient is beyond thrilled. The lack of eyebrow hair previously was a constant reminder of his disease process and the new eyebrows have given him a new self confidence.

Here is a patient from Asia who underwent an eyebrow transplant procedure with a local surgeon. Unfortunately her results were very unnatural. She had “cobble stoning” in the eyebrows (usually from incorrect grafts placement) as well as a very small amount of growth and unnatural growth of the hairs.

I placed approximately 450 grafts into each eyebrow over 2 procedures and the results are shown below.

The patient is thrilled with her results and states that her eyebrows have given her a renewed self confidence.

It is imperative to chose a surgeon with extensive eyebrow transplant experience as this is a very nuanced procedure that requires a tremendous amount of artistic vision as well as technological expertise.
