Hair Transplant Grafts Placed Into Scarring Alopecia

Today I will discuss a patient of mine who presented with a form of scarring alopecia called Lichen Planopilaris. This resulted in the loss of all the hair follicles in the frontal and mid scalp as well as the eyebrows and sideburns. The patient initially thought that the hair loss was androgenic alopecia, but after my examination and subsequent scalp biopsy the diagnosis was confirmed. It is imperative that the hair transplant surgeon has a thorough knowledge of all the forms of alopecia as different causes result in different outcomes and some are candidates for hair transplants and others not.
In this case the biopsy also showed that the scarring alopecia had “burnt out” and was no longer active. We decided to do 3 small areas with test grafts to see if they would grow before considering a more expansive procedure. What we found was excellent growth in all the test areas after only 12 weeks. After discussing with the patient and letting him know that there still is a chance that we won’t see the same growth rate that is seen in healthy scalp, and the patient decided to proceed with a more extensive procedure.

The grafts were placed into the patient’s previous strip scar (which was created by another doctor), and the frontal scalp and hairline. Beard FUE Into Previous Hair Transplant Strip Scar

The patient in this video had a wide donor strip scar performed by another physician. The physician tried to resect the scar multiple times with no improvement. The patient also had scalp micro pigmentation which only provided slight improvement. I harvested approximately 500 beard grafts via FUE and placed them into the scar. The result show 2 weeks after the procedure shows significantly visible improvement and it will get even better when the hairs grow longer. In addition there is no visible scarring in the heard area from where the grafts were taken.

Hair Transplant Comfort During The Procedure

Today I want to discuss comfort during the hair transplant procedure. Since I have had multiple hair transplant procedures myself I understand exactly how it feels to sit in the hair transplant chair for 5-8 hours for a procedure. Because of this, I designed a custom memory foam cushion to sit on top of my exam chairs to provide the maximum amount of comfort during the procedure. This makes the chair feel like a soft bed as opposed to a typical exam chair, which is not necessarily made with the idea of sitting in it for hours at a time. The feedback from this memory foam cushion has been amazing and it’s just one more small detail that makes the entire day of the hair transplant procedure that much more comfortable. My thinking is that the day of the procedure should feel more like a day at the spa as opposed to an uncomfortable experience.

Engineering Organs- Is Hair Cloning Next?

Today I want to highlight a new article I just saw on CNN discussing how various organs were engineered in the lab to create new organs for individuals that were missing these specific organs.
This is a huge step forward for organ cloning as the closer we get to being able to clone tissue, the closer we get to being able to clone hair. Once we can clone hair we will no longer need to harvest donor hair from the scalp and every person will have unlimited supply for whatever demand they have for scalp hair. I am not trying to say that we are a few years away from being able to clone hair in practical terms, but I know think that we may see hair cloning in the real world in 10-20 years.
I can’t wait for that day!

Eyebrow and Eyelash Transplant Article

Here is a link to a recent article from the UK press about a woman who underwent an eyebrow and eyelash transplant. Eyebrow transplants have recently become much more common as more people have discovered this procedure exists. Eyelash transplants are much more uncommon. Because the hair is taken from the scalp it will grow longer and need to be trimmed. This poses 2 problems for eyelash transplants. Firstly the hair can grow into the eye, thus scratching the cornea. Secondly, because you will need to trim the eyelash hairs every few weeks there is a high risk of injury to the eye. In addition, because the eyelid is so thin, and has so many muscles and nerves in a small area, there is a very high incidence of complications in eyelash procedures. It is for all these reason that I do not perform eyelash transplant procedures.
On the other hand, Eyebrow transplant procedures in the hands of an experienced eyebrow transplant surgeon can be very successful and I have been performing these procedures for over 10 years. In the patient highlighted in this article, she has a tremendous amount of eyebrow pencil makeup on her eyebrows in the “after” photo, thus not really giving an accurate image of what her transplanted eyebrows really look like.

With all the recent press surrounding beard transplants I wanted to highlight a patient of mine. This patient had 2 bald patches in his beard in the same spot on both sides. This is genetic and a very common phenomenon. In this case I harvested the donor hair from his neck beard via FUE and transplanted the grafts into the bald patches. The result is shown 10 months later. The patient was very happy with his results.
