Today I want to discuss a very new exciting development. For the past year I have been prescribing a compounded topical prescription formula that contains Dutasteride, Finasteride, Minoxidil, Retin A, and Triamcinolone. Dutasteride and Finasteride (propecia) are potent DHT blockers. Minoxidil is a vasodilator which has been proven to help genetic hair loss. Retin A helps with absorption and triamcinolone helps with scalp irritation. I call the formula “hair defense” as it’s defending the native hair. It does not grow back dead follicles that have been lost. It does however prevent susceptible follicles from falling out, and can turn miniaturized follicles (follicles that are on their way out and have become cosmetically insignificant) back into healthy terminal follicles, which can then give the appearance of greater density.

Below, you can see examples of patients of mine who haven been on the topical at least 6 months and all the photos shown below are just with the topical only as treatment.

This is not a substitute for a hair transplant procedure. Rather it is a an adjunct meant to preserve the native hair so that the results of the hair transplant procedure look better for longer. In some cases it can be used as an intermediate treatment meant to slow or stop the continuing loss before the transplant is indicated.

Finally, one of the best things about the topical is that the incidence of sexual side effects (though low in frequency can still occur in 1-2% of patients taking oral finasteride) are even more uncommon with this topical as the systemic absorption of the medication is less than the oral medication.

I have seen a profound impact of this topical medication on my own scalp and hundreds of my patients.

For a small number of patients that may get irritation from any of the components, the formula is customizable and components can be pulled out in any amount.

Here is a patient who presented to me after undergoing 2 previous procedures with another physician. He was having constant pain in the  donor region. On examination his strip scar was raised and 7-9mm wide and in certain spots there were 2 separate scars.

I decided to revise the scar in the context of the patients procedure to increase the density in his hairline.

While I was extracting the old scar I noticed that there were a number of retained nylon sutures in the wound that were left there and had never been removed. These were likely contributing to the pain and poor healing. I removed the retained sutures and revised the scar closing with a triuchopytic closure to get hair to grow through the scar.

I then placed 1003 grafts into the hairline.

The results of the hair transplant and the donor scar revision are shown below.

Today I will discuss the difference in hair type for hair transplants. Based on studies 0.1mm increase in the diameter of the hair follicle can add up to 30% increased cosmetic density. This means that the greatest factor affecting cosmetic density is the thickness of the hair follicle. People with coarse hair will have a much better cosmetic result than people with fine hair with the same number of follicles. In addition, curly hair will yield a better cosmetic result than straight hair. Also, the less color contrast between the hair and the scalp (grey hair on light skin versus dark hair on white skin) the better the cosmetic result.

The analogy I typically use it to imagine a forest of trees in front of you. If the trees are thick with large wavy branches you will see through less of the forest than thin trees with small flimsy branches. This is the same with hair. Coarse curly hair will always yield a better cosmetic result than fine straight hair.

This is not to say that people with fine straight hair are not good candidates for the procedure, on the contrary they can be excellent candidates. Prospective hair transplant patients need to know this information in order to prepare themselves for realistic expectation of the outcome of their hair transplant procedure.

Here is a patient of mine who just returned for his 9 month follow up after receiving 1372 grafts to the hairline via FUT. The objective was to fill in and define the existing thinning hairline.

The patient is already thrilled with his results and he should see some additional cosmetic density over the next 3-6 months as the transplanted hairs thicken in diameter.

I am often asked what you look like 7 days after an FUE hair transplant so here are the images.

This patient received just over 1500 grafts to the frontal and mid scalp via FUE using a 0.85mm extraction tip.

As you can see with this small extraction tip the donor zone heals very well and there is basically no sign of any harvest after 7 days.

Here is a patient of mine who received old hair transplants with larger grafts from another physician many years ago. He came to me requesting greater density and more irregularity in the hairline to create more naturalness. He was not a candidate for another strip procedure and he had very limited donor reserves on his scalp.

I extracted approximately 600 grafts from the sides of his scalp via FUE and just over 600 grafts from his neck via FUE as well. I used a 0.8mm punch tip for my extractions so that the extraction sites would not be visible. The neck grafts were placed in the frontal and mid scalp for greater density and the scalp grafts were placed in the hairline to create a more natural look overall.

The results are shown after 12 months.

Beard hair FUE is an excellent option for patients who do not have any scalp hair available and this case demonstrates that very well.
