I was on the fence regarding hair transplant, and an now over one year after the procedure. The changes are gradual, and only those who I told I had the procedure could tell I had it. Interestingly, I’ve received other comments regarding my appearance, for example, asking if I’ve lost weight. I haven’t lost weight, but could not be more pleased with my decision to undergo the hair transplant.

Dr. Dauer should be your first and last stop for hair transplantation. Being a fellow MD, albeit in a different specialty, the candor and professionalism offered by Dr. Dauer was reassuring, as I had preconceived notions what would be best for me in that I believed FUE would be ideal. Dr. Dauer patiently and thoroughly went over my options after his examination, and we decided on the strip harvest procedure. He is an expert with both, and I could not be more pleased with the decision to go with this option and the results. The process is gradual, and having a follow up with Dr. Dauer a bit over a year reminded me of the significant improvement in density, coverage and fullness when I viewed by pre-procedure photos and my scar is pretty much invisible. Beard FUE Grafts To Cover Strip Scar and Add Density

Here is a patient who came to me after having undergone several previous procedures and scalp reductions with other hair transplant physicians.

He wanted to add hair to his mid scalp and crown (where there was virtually no hair and a scalp reduction scar) and to attempt to conceal a very wide donor scar from his previous strip hair transplant procedures.

I extracted just over 900 grafts from the neck beard and placed them into the scalp and scar. The results are shown after 9 months and they are already impressive.

The patient should see some increased density over the next 3-6 months as the hairs thicken in diameter.

Although the beard hair is a different consistency than scalp hair it works well in cases like this where there is no more scalp hair to donate and when “filler” hair is needed for increased cosmetic density.

The patient is looking forward to another procedure to add more density in the scalp from the beard hair.

A patient with a relatively thick beard can have up to 2000 grafts to donate from the area below the jaw line.

FUE versus Strip Harvest (FUT) Hair transplant Procedures

In the most recent issue of the International Society Of Hair Restoration Surgery journal there is an extensive very interesting debate between those who advocate FUT over FUE and those who advocate FUE over FUT.

Many of the salient points discussed I have previously written about here in my blog. I will briefly review a few of the most important points here.

FUE Advantages- less invasive, diffuse dot scarring versus strip scarring with FUT which in most but not all cases allows the patient with FUE to cut the hair down to a #1 or #2 without showing any evidence of scarring, less discomfort post procedure

FUT Advantages- no need to shave the head to harvest the donor hair, ability to harvest more hair in a single day, more donor hair available to harvest throughout the patient’s life (start with FUT and convert to FUE after FUT is no longer possible thus maximizing the donor zone overall), less expensive than FUE

There are many subtleties to the advantages and disadvantages of both of these procedures.

What is most important is that your hair transplant surgeon is proficient in both procedures and offers both procedures to patients.

I prefer to educate my patients as to the benefits and drawbacks of both FUT and FUE and let them choose the procedure that suits them best. I enjoy performing both FUT and FUE and let patient’s know that if everything goes as planned they should have a great result no matter which method they choose.

Hair Transplant Patient With Scarring Alopecia

This is a very interesting patient of mine. This patient, whom both he and his wife happen to both be physicians, presented to me complaining of hair loss in the frontal scalp, sideburns, and eyebrows over a number of years. The patient denied any other medical conditions.

On examination I determined that the patient most likely had a form of scarring alopecia and referred the patient to a local dermatologist for confirmation. A biopsy confirmed Frontal Fibrosing Alopecia and the pathology revealed that the disease appeared inactive.

After discussion with the patient and the treating Dermatologist we agreed that I would transplant a small number of test grafts in different areas of hair loss and we would see how they grew before proceeding with a larger procedure.

The test grafts grew well and the patient returned requesting a much larger procedure to create cosmetic density in the frontal scalp, sideburns, and eyebrows.

I explained to the patient that even if we were successful with the transplant procedure, there was always the possibility that the disease could recur in the future and possibly destroy the transplanted grafts. The patient understood and wished to proceed with the procedure.

The patient received 2521 grafts via FUT to the frontal scalp, mid scalp, and sideburns, and approximately 250 grafts per eyebrow. Care was taken not to pack the grafts too densely because of possible vascular compromise of the tissue to be transplanted secondary to the scarring alopecia process.

The results are shown after 1 year.

The patient is absolutely thrilled with his results and is now requesting a second procedure to increase the density.

Here is a patient of mine who I perfumed a hair transplant of 2258 grafts via FUT. The patient has very fine, straight hair, which is the least ideal hair type when trying to maximize cosmetic density, but despite this the results are still significant and the patient is pleased.

The patient also had a previous procedure with another physician and discusses the difference between my procedure and his previous one.

Hair Characteristics In Hair Transplants

Today I will discuss the difference in hair type for hair transplants. Based on studies 0.1mm increase in the diameter of the hair follicle can add up to 30% increased cosmetic density. This means that the greatest factor affecting cosmetic density is the thickness of the hair follicle. People with coarse hair will have a much better cosmetic result than people with fine hair with the same number of follicles. In addition, curly hair will yield a better cosmetic result than straight hair. Also, the less color contrast between the hair and the scalp (grey hair on light skin versus dark hair on white skin) the better the cosmetic result.

The analogy I typically use it to imagine a forest of trees in front of you. If the trees are thick with large wavy branches you will see through less of the forest than thin trees with small flimsy branches. This is the same with hair. Coarse curly hair will always yield a better cosmetic result than fine straight hair.

This is not to say that people with fine straight hair are not good candidates for the procedure, on the contrary they can be excellent candidates. Prospective hair transplant patients need to know this information in order to prepare themselves for realistic expectation of the outcome of their hair transplant procedure.

FUE Hair Transplant To Hairline

Here is a patient of mine who received just over 1100 grafts via FUE to the hairline and frontal scalp. The grafts were extracted manually using a 0.8mm extraction tip.

The grafted region and the donor regions are shown 1 week post procedure and as you can see there is almost no evidence of scarring in the donor region and very little to be seen in the recipient region other than the new transplanted grafts.
